Complete the Seattle Children's Hospital lock box survey

Seattle Children’s Hospital and Prevention WINS Coalition have developed a survey to assess medication storage methods and options in Washington State in hopes of promoting locked medication storage for families. Please complete and disribute the SCH Medication Lock Box Survey.

We will be using the results from this survey to further our research and gain insight on how to develop medication lock box education and outreach. The responses from this survey will be completely confidential and anonymous. The survey will be open from July 15^th-26^th, 2019.
