Submit Your GOT Outcomes! Awards Application Due August 1st

The deadline for CADCA’s GOT Outcomes! Awards is only a couple weeks away!  Now is the time to make those adjustments and updates that will truly demonstrate the hard work you’ve put into making CADCA’s 5 products unique to your community.  We are very excited to hear your story and the opportunity for other coalitions to be inspired.  As a quick reminder, these are the documents that are required to be sent in:  

1. Completed Application Form 

2. Community Assessment  

3. Logic Model  

4. Strategic and Action Plan 

5. Evaluation Plan  

6. Evaluation Communication Plan  

7. Sustainability Plan 

8. One-page description of Coalition Outcomes (short-term, intermediate, and/or long-term)
