Presented at 2018 All Provider Meeting in Yakima. Use this template as a guide for telling your prevention story with data. Page 1 provides a sample, and page 2 provides more information on how to build a document like this using reports in Minerva and other sources of information. While the template has a specific prevention provider type as an example, this format may be of use for all of our prevention providers.
