Healthy Youth Survey: Question revision and new question proposal

The Healthy Youth Survey Planning Committee (HYSPC) is now collecting proposals for new and revised questions to include in the 2025 Healthy Youth Survey (HYS). If you would like to submit a proposal for a new question to be added to the survey, or you would like to propose a revision to an existing question or question scale, please complete and submit this form no later than Friday, April 12, 2024.

If you are interested in submitting a topic for consideration on the 2025 HYS, but you would not like to submit a full proposal for specific questions, you can submit general suggestions via this shorter form. 

To learn more about the Healthy Youth Survey, look at past results, and view the questions included on the 2023 survey, please visit the HYS website.

Please save these dates for 2025 HYS revision meetings:

  • Wednesday, May 22, 8 a.m. to 4:30 p.m.
  • Thursday, May 30, 8 a.m. to 4:30 p.m.

Note: Revision meetings are open to community partners. Details and meeting links will be forthcoming.
